I came across this photo on the kansascity.com website of Chris Dwyer on the mound this spring training. I see the dreaded "inverted W" that I believe has been blamed for pitchers needing Tommy John surgery. The key is that both elbows are above the shoulders.
Compare to the deliveries of CJ Wilson and Steven Strasburg, both of whom are coming off Tommy John surgery:
